Service Description
Establish care for weight loss with the medical professionals at Restorative Health and Wellness. Our New Weight Loss Patient Broken Arrow program offers a comprehensive first visit for $99, plus the cost of your prescription. If you have a recent lab panel from your primary care physician, bring it to your appointment for review. If not, we can order one.
Cancellation Policy
1. The first visit is $99 PLUS the cost of your prescription. 2. If you have a lab panel from your PCP in the last 6 months, bring it to the appointment and we will review it/scan it into your chart. 3. If you don't have a recent lab panel, you can opt for a panel we order for $65 OR we can order a panel at DLO and you can use your insurance. You will be responsible for any charges not covered by insurance if you opt for DLO. 4. Follow up visits are conducted monthly either in person or via telemedicine. 5. You can request a lab panel any time during your care. You will be responsible for the cost of the lab panel. If you wish to utilize your insurance, we can order your panel at DLO. You will be responsible for any charges not covered by insurance if you opt for DLO.
